If you've spent more than ten minutes on wellness TikTok lately, you've probably seen someone holding a vial or a supplement bottle and explaining that NAD+ is the molecule your cells are running out of, and that topping it back up will basically pause your biological clock. The claims range from 'more energy in three days' to 'my inflammation is gone' to the more ambitious 'I reversed my age by ten years on a blood test.' Reddit threads in biohacking communities go further, with users stacking NAD+ precursors alongside mitochondria-targeting peptides and reporting outcomes that sound more like a clinical trial than a personal experiment.

The specific compounds that come up most often are nicotinamide mononucleotide (NMN), nicotinamide riboside (NR), and peptides like MOTS-c, which is described in those threads as an 'NAD+-linked mitochondrial peptide.' Creators often blur the line between these compounds, treating them as interchangeable parts of one anti-aging system. They are not interchangeable, they work through different mechanisms, and the evidence behind each one sits at very different tiers. That blurring is where a lot of the hype gets its fuel.

What Does NAD+ Actually Do in the Body?

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every cell. It plays a central role in energy metabolism, specifically in the electron transport chain that mitochondria use to produce ATP. It also acts as a substrate for enzymes called sirtuins and PARPs, which are involved in DNA repair and gene expression regulation. NAD+ levels do appear to decline with age in human tissue, a finding supported by studies including a 2012 paper in Cell by Gomes and colleagues that showed age-related NAD+ decline in mouse muscle tissue and its connection to mitochondrial dysfunction.

That mouse study is important context because it is one of the foundational papers that launched a thousand supplement brands. The leap from 'NAD+ declines in aging mice' to 'supplementing NAD+ precursors in humans reverses aging' is a large one, and the research has not fully bridged it. Human cells do absorb NMN and NR and convert them to NAD+, and some human trials have confirmed that blood NAD+ levels rise after supplementation. What those elevated levels actually do for human health outcomes is a much harder question to answer.

What Do the Human Studies Actually Show?

The honest answer is: some interesting early signals, nothing close to the claims on your feed. A 2020 randomized controlled trial published in Nature Communications (Yoshino et al., 60 postmenopausal women with prediabetes) found that 250 mg per day of NMN over ten weeks improved muscle insulin signaling and glucose uptake compared to placebo. That is a real finding in a real RCT. It is also a specific metabolic outcome in a specific population, not a general anti-aging result.

A 2023 trial published in GeroScience (Yi et al., 80 healthy middle-aged adults) looked at NMN supplementation and found improvements in muscle strength and performance on a six-minute walk test. Again, a real signal, a modest one, in a relatively small trial. What neither of these studies showed was reversal of biological age, elimination of inflammation, or the kind of whole-body transformation creators describe. The studies that do exist are mostly short-term, small, and focused on narrow biomarkers. Long-term safety and efficacy data in humans is still limited.

NR has a similar profile. A 2018 study in Nature Communications (Martens et al., 24 healthy older adults) found that NR supplementation raised blood NAD+ levels and showed some reduction in aortic stiffness, a cardiovascular marker. Interesting. Preliminary. A sample size of 24 is not the foundation for a longevity protocol, and the researchers said as much.

Where Do Peptides Fit Into This?

This is where the TikTok narrative gets genuinely muddled. MOTS-c is a mitochondria-derived peptide encoded in mitochondrial DNA, and it does interact with NAD+ metabolism pathways. Research in mice has shown that MOTS-c injections improved insulin sensitivity, reduced obesity-related metabolic dysfunction, and extended lifespan in some aging mouse models. A 2021 paper in Nature Aging (Kim et al.) showed that MOTS-c levels decline with age in humans and that higher circulating MOTS-c correlated with better metabolic health in older adults. That correlation study involved human data, but correlation is not causation, and it is not a trial of MOTS-c as an intervention.

As of this writing, there are no published human RCTs testing MOTS-c as an administered compound for anti-aging or metabolic outcomes. The existing human evidence is observational. The intervention evidence is in animals and cell cultures. That is a preclinical evidence base, and creators who describe MOTS-c as a proven NAD+-linked anti-aging peptide are skipping over that distinction entirely. MOTS-c is a research compound. It is not FDA-approved for any use.

Other peptides sometimes lumped into NAD+ conversations, like Epitalon, have an even thinner human evidence base. Epitalon has been studied in small Russian trials going back to the 1990s, but those studies have not been replicated in large, well-controlled Western trials, and the methodology of the older work is difficult to evaluate by current standards. Grouping Epitalon with NMN and calling it all 'NAD+ peptide therapy' is a category error that makes the whole stack sound more validated than any individual piece of it is.

So What Does the Evidence Actually Support?

NMN and NR are the most studied compounds in this space, and the human evidence, while early, does suggest they raise NAD+ levels in blood and may have modest effects on specific metabolic markers in certain populations. That is genuinely interesting science. It is not 'reverse aging in 30 days.' The gap between those two things is where supplement brands and content creators have built entire businesses.

The peptide side of this conversation, particularly MOTS-c, is almost entirely animal and cell-culture data right now. That doesn't mean the research isn't worth following. It means the claims being made on social media are running about ten years ahead of the clinical evidence. Readers who are curious about this area are better served by watching what comes out of ongoing registered trials than by taking creator testimonials as a substitute for that data.

One more thing worth flagging: some products sold online as 'NAD+ peptides' or 'NAD+ boosting peptide blends' are not regulated as drugs. The FDA does not approve supplements for anti-aging claims, and the research compounds in this space are not approved for human use outside of clinical trials. If a product is being marketed with disease-treatment language or dramatic outcome guarantees, that is a regulatory red flag, not just a hype problem.
